Job SummaryWe are seeking a highly skilled Care Manager to join our team at Oklahoma Complete Health. As a Care Manager, you will play a critical role in developing and implementing care plans for our members with complex behavioral health needs.Key Responsibilities

• Evaluate member needs and develop personalized care plans to ensure high-quality, cost-effective healthcare outcomes.
• Collaborate with community resources, providers, and specialists to ensure timely access to needed care and services.
• Monitor member status and outcomes, making recommendations to care plans as needed.
• Provide education to members and their families on procedures, healthcare provider instructions, treatment options, referrals, and healthcare benefits.
• Comply with all policies and standards, including state, federal, and third-party payer regulations.Requirements
  • Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
  • 2-4 years of related experience, including experience with children/youth in foster care and/or juvenile justice.
